How to prepare for your visit

In order to help make your visit a pleasant experience, we are going to ask you to supply a few things to us in advance. We will be happy to retrieve any document from your past medical history.

We will be happy to retrieve any document from you past medical history such as Echocardiogram, EKG,
Cardiac Catheterization that will help your Cardiologist better assess your condition or concern at your
visit, whether the records are from a past hospital stay, an emergency visit, or records from your
Primary Care Physician. This will require you to sign a Release of Medical Records form. Please
complete the form and send to the office of your scheduled appointment.


If your medical insurance requires a referral, you will need to get in touch with your Primary Care
Physician to let them know when your scheduled appointment is and ask them to enter a referral for
you. You will need our NPI number for many of the insurances. If you are making an appointment for
the CCP office the NPI number is 1649272386 , for the PENN offices the NPI number is 18910242826.
