Coordinate care and provide clinical assessment services to CCP patients
Communicate with hospital care management to ensure CCP is involved in all discharge decisions
Build a relationship with patients and physicians to coordinate a plan of care including appointments, education, provision and/or enablement of support services and representation within the multidisciplinary care environment.
Assume responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each patient including support for interdisciplinary and cross facility collaboration.
Perform outpatient clinical responsibilities including but not limited to Coumadin management, triage, and follow up phone calls
Current RN licensure for the state from an accredited college
BSN in Nursing

Demonstrates sound knowledge and actions in the care and decision making for designated patient populations and seeks guidance appropriately.

Must possess the ability to utilize a clinical reasoning process for assessing, planning, implementing and evaluating the plan of care while ensuring that coordination of services are done in a timely manner.
